When adding a new disk to an ASM disk group I found following error.
Failed to commit: ORA 15032: not all alterations performed
ORA 15075: disk(s) are not visible cluster-wide
I looked back and found that the scandisks was not done on all the nodes of cluster.
On all the database nodes did following.
sudo /etc/init.d/oracleasm scandisks
[sudo] password for oracle:
Scanning the system for Oracle ASMLib disks: [ OK ]

Tried adding disk again and it went through smoothly.
